When you participate in Smile Makeover Dental Grant Program you are eligible to win Free Teeth Whitening Treatment ($500 value) or $10,000 Dental Grant Prize.

• Anyone can enter through DentalConnectionsUSA.com with no purchase necessary. 
• Person may reenter this program once per month. 
• Top vote getter will be selected to win a Free Tooth Whitening through the "Vote for Me" campaign. 
• An additional winner is selected by random drawing through smilewinner.com. To obtain a code for smilewinner.com you must be a patient of record with your sponsoring dentist.
• Smilewinner.com patients must have visited a dentist's office within 6 months prior to the drawing date.  
• Winning patient receives no cash value. A Check in the full amount of the award is made out to the dentist and mailed to the winner. The patient simply has a credit balance on account in order to receive the dental benefit awarded: $500 for tooth whitening award and the full cash value of the cosmetic dental award specified for that particular award date. 
• The winning patient must agree to allow us to announce their name as a winning patient of our grant program. The winning patient also agrees to provide before and after photos to display the dental results and gives us permission to use photos for our own promotional purposes to advance the cause of this program.
